Registration for the 2021-2022 season is now open!

NOTE: registration is on-line by credit card only at this time. If you are unable to use a credit card for payment, please contact the MDHA registrar and alternate arrangements can be made. Registration instructions for new and returning players below. Please read carefully.
New to MDHA : Welcome!
First Year Playing Hockey?
First year players (never registered in any association before) must reside within the MDHA boundaries, complete the online form below and provide copies of proof of age (Birth Certificate) and proof of residency (recent utility bill, hydro or gas). A child may only be registered by the custodial parent with whom he/she resides (4 or more days a week)*. First year players are asked to follow the process below. Using this information a profile will be created in the Hockey Canada database. Once this is done players must register for the season following instruction that will be provided by the registrar. Even though U7 season registration is not yet open, new U7 players are encouraged to provide this information as soon as possible.
Note: For all new players, a one hour, on-line Respect in Sport Parent Course is to be completed by at least one parent in the household at a cost of $12 + tax.
Played before Elsewhere?
If you have moved into the MDHA boundaries and have played hockey before, you must submit the following documents. A child may only be registered by the custodial parent with whom he/she resides (4 or more days a week)*:
- HEO Attestation of InterBranch/Association Residency Form: Appendix A
- Proof of Residency that includes:
- A copy of the Purchase and Sale Agreement OR Lease Agreement for
the new residence (1-year minimum lease), AND
- Any two of the following documents:
• School Registration*
• Utility Bill or Cable/Internet bill (no cell phone) that matches new
• Driver’s License (parent) that matches new address
• Vehicle insurance/registration that matches new address.

*In accordance with Hockey Canada and HEO Minor regulations, the MDHA reserves the right to view separation agreements in the case of court sanctioned custody.
Returning Players : Welcome Back!
For the 2021-2022 season, ensure you have created your new Spordle (HCR 3.0) User Account prior to attempting registration. Instructions for creating this account can be found on the Spordle (HCR 3.0) User Account Creation Instructions page under the Registration tab.
