Privacy Policy
MDHA is committed to the protection of its membership’s personal information during the course of delivering its minor hockey program. This privacy policy and procedure is in accordance with the Personal Information Protection and Electronic Documents Act (PIPEDA) and under the direction of Hockey Eastern Ontario (HEO).
PIPEDA is an Act to support and promote electronic commerce by protecting personal information that is collected, used or disclosed in certain circumstances, by providing for the use of electronic means to communicate or record information or transactions and by amending the Canada Evidence Act, the Statutory Instruments Act and the Statute Revision Act.
The MDHA will comply with PIPEDA with respect to the personal information it collects by ensuring:
1. Accountability
MDHA is responsible for the personal information under its control and has designed the Risk & Safety Direction to oversee compliance to the MDHA Privacy Policy and Procedure and PIPEDA.
2. Identifying purposes for collection of personal information
MDHA will identify, before or at the time the personal information is collected, the purpose for which the personal information is being collected. The purpose may be stated orally or in writing, depending upon the circumstances and the sensitivity of the information.
3. Obtaining clear consent
MDHA will obtain the individual’s consent for the collection, use or disclosure of his or her personal information, except where as permitted by law.
4. Limiting collection of personal information
MDHA shall limited the collection of personal information that is necessary for the purposes identified by the MDHA at the time of collection, or as otherwise permitted by law.
5. Limiting use, disclosure and retention of personal information
MDHA will not use or disclose personal information for the purposes other than the identified purposes for which it was collection, except when the consent from the individual is obtained or as permitted by Law. MDHA will retain personal information only for as long as necessary to fulfill the purpose identified when consent was obtained.
6. Accuracy of personal information
MDHA will keep the personal information it collects as accurate, complete and up-to-date as is necessary for the purposes for which it is to be used so as not to make decisions based upon inaccurate information.
7. Security
MDHA will safeguard the personal information it collects from authorized access and use and will destroy the information when it is no longer needed for the original purpose.
Pursuant to PIPEDA, “personal information” is information about an identifiable individual that includes any factual or subjective information, recorded or not, in any form.
